If you wish to register click on the links below to open the forms. When you have completed ALL of the details, click on the "Send" button to email your forms to us.

Online medical questionnaire for new patients
Please ensure BOTH forms are filled in correctly, incomplete forms cannot be processed. If you need help filling in the forms, please contact the surgery.
In addition to the above forms, you will need to email 2 forms of ID, these need to include proof of current address and preferably photo ID to: syicb-doncaster.regentsquarereception@nhs.net.
A member of staff will then contact you to book a new patient check with one of our health care assistants.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
